Cost-effectiveness of antihypertensive treatment. General considerations.

چکیده
Cost-effectiveness analysis, a set of methods for estimating the costs and health effects of medical interventions, helps decision makers to compare alternative treatments and to decide which interventions are appropriate in which situations. This article outlines some fundamental principles of cost-effectiveness analysis and discusses the characteristics necessary if alternative therapies are to be more cost-effective than traditional drug treatment for hypertension. The discussion focuses primarily on the newer drugs like angiotensin converting enzyme inhibitors, alpha-adrenergic receptor blockers, and calcium antagonists.

An economic evaluation of antihypertensive therapies based on clinical trials
OBJECTIVE Hypertension is a major issue in public health, and the financial costs associated with hypertension continue to increase. Cost-effectiveness studies focusing on antihypertensive drug combinations, however, have been scarce. The cost-effectiveness ratios of the traditional treatment (hydrochlorothiazide and atenolol) and the current treatment (losartan and amlodipine) were evaluated i...

OBJECTIVE To determine which treatments for preventing coronary heart disease should be offered to which patients by assessing their incremental cost effectiveness. DESIGN Modelling study. DATA SOURCES Cost estimates (for NHS) and estimates of effectiveness obtained for aspirin, antihypertensive drugs, statins and clopidogrel. DATA SYNTHESIS Treatment effects were assumed to be independen...
متن کاملRational Prescribing in Primary Care (RaPP): Economic Evaluation of an Intervention to Improve Professional Practice
BACKGROUND Interventions designed to narrow the gap between research findings and clinical practice may be effective, but also costly. Economic evaluations are necessary to judge whether such interventions are worth the effort. We have evaluated the economic effects of a tailored intervention to support the implementation of guidelines for the use of antihypertensive and cholesterol-lowering dr...
متن کاملEffectiveness, safety and cost of drug substitution in hypertension
Cost-containment measures in healthcare provision include the implementation of therapeutic and generic drug substitution strategies in patients whose condition is already well controlled with pharmacotherapy. Treatment for hypertension is frequently targeted for such measures. However, drug acquisition costs are only part of the cost-effectiveness equation, and a variety of other factors need ...

In this issue, Palmer et al. [1] demonstrate that screening for nephropathy in hypertensive type 2 diabetic patients and subsequent treatment with renoprotective antihypertensive agents may result in excellent value for money from the US health care perspective. Estimated costs and effects were combined in a cost-utility analysis, to express the incremental costs per quality-adjusted life year ...
